As far as I know the digipacks were only released in US and the jewel cases ind the UK

The US commercial cd single came in the regular sized jewel case with black tray.

The US promo cd came in a digipak.

The UK commercial cd came in a digipak.

The UK promo cd came in a slimline jewel case.
